Practitioner: Dr. Ulrich Volz & Dr. Rebekka Hueber Location: SWISS BIOHEALTH CLINIC | Kreuzlingen Content: In this surgery, a full-mouth rehabilitation of the maxilla and mandible with 20 SDS ceramic implants using the compatible navigation system of 2INGIS®, as well as a bone augmentation with bone shell technique and a Bone Implant Stabilization System (BISS) – osteosynthesis plate in region 33, is performed by oral surgeon Dr. Rebekka Hueber. The procedure is performed under intubation anesthesia. To ensure optimal access for the navigation system, the splints were divided quadrant-wise. In region 26, 27, Dr. Ulrich Volz performs a bone augmentation with the Bone to Implant Stabilization System (BISS) and the open healing technique to horizontally expand the soft and hard tissue of the narrow alveolar ridge and achieve an optimal peri-implant tissue situation.

Practitioner: Dr. Josephine Phillips Location: SWISS BIOHEALTH CLINIC | Kreuzlingen Content: The subject of this visit is two large horizontal and vertical bone augmentations using the BISS system (bone to implant stabilization system) in the 2nd and 3rd quadrants, simultaneous external sinus floor elevation of the 2nd quadrant and implantation 35. Tooth 35 was root-treated and 36 had a very large inflammation, preservation was ruled out. In region 35, immediate implant placement of a one-piece SDS_1.2_4611-ov was possible, and the implant was directly restored with a long-term provisional. In region 36/37 and in the 2nd quadrant, a two-stage procedure was selected to provide a secure basis for the implant restoration in the long term, since the minimum bone height (in some cases 1 mm) was too low. bone height (in some cases 1 mm) did not allow immediate implant placement. Thanks to the BISS system, the dimensioning of the abutment could be individually adapted and minimally invasive augmentation could be performed within a short time by means of two 2-unit cages. The materials used were the 2-bite cages, autogenous bone, Allodyn, BioCollagen, PTFE membranes and PRF matrices. After surgery, the patient was doing great, had no swelling or pain, and we look forward to seeing him again for cage removal and later implant placement.

Practitioner: Dr. Ulrich Volz Location: SWISS BIOHEALTH CLINIC | Kreuzlingen Content: Part 1 In this exciting live observation, you will see step-by-step a three-dimensional bone augmentation in region 15-16 with the BISS and an external sinus lift, performed by Dr. Ulrich Volz. Due to a pronounced cyst on tooth 15, the alveolar ridge was augmented both vertically and horizontally with a double star, combined with an external sinus lift in region 16. A multilayer technique was used consisting of allogeneic bone grafting material in combination with a xenogeneic hybrid material (Purgo), collagen membranes and PRF, as well as a temporary sealing PTFE membrane.
Part 2 Due to severe chronic inflammation on teeth 24, 25 and 26, which were not worthy of preservation, the patient had a bone defect in both vertical and horizontal dimensions, which was initially augmented by Dr. Ulrich Volz with a double star of the BISS in combination with an external sinus lift. Allogeneic and xenogeneic hybrid material (Purgo) was used as bone substitute in a layering technique. In the spirit of the open healing technique according to Prof. Ghanaati, this was covered with collagen membranes, PRF matrices and a PTFE membrane to achieve not only a gain in hard tissue but also in soft tissue.

With the help of the BISS augmentation system, the basis for subsequent implant placement was created during this visit. In this case, we used Allodyne Sticky Bone and Biocollagen membranes as well as a Cytoplast membrane and an individualized 1er – BISS Cage. In today’s visit, we will present immediate implant placement 15, 16, 17 and late implant placement 26, each with internal sinus floor elevation. The clearly apically inflamed teeth 15, 16, 17 were gently removed with maximum bone preservation and directly replaced with ceramic SDS immediate implants. In region 26, a reduced bone volume was available after previous extraction (external dentist), which was utilized to the maximum in order to restore the patient in a minimally invasive and single-stage procedure.

Practitioner: Dr. Paul Kilanowski Location: SWISS BIOHEALTH CLINIC | Kreuzlingen Content: If there is both a dysfunction of the habitual bite position and multiple chronic inflammations in the jawbone of our patients, we prefer a step-by-step procedure in which the absence of inflammation is first ensured by means of a surgical procedure in order to then gently guide the patient into a physiological bite position before the final prosthetic restoration. In this observation session, Dr. Paul Kilanowski will show you step by step the restoration and preparation of ceramic implants that have already been placed and healed with long-term temporaries, analogous to the restoration with final ceramic crowns. Implant 23 was retightened in this session. Furthermore, you can see the preparation of the patient for functional therapy with a MAGO splint in the course of centric bite registration. In the initial findings of this patient case, there were multiple chronic inflammations on the root canal-treated teeth 16, 12, 11, 22, 23, 27 and 47, as well as chronic peri-implantitis on implant 46. In one surgical session, Dr. Rebekka Hueber placed ceramic implants in regions 16, 12, 11, 21, 22, 23, 26, 46 and 47 and remediated all chronic inflammatory processes. Now you can see the healed situation and the first step of the patient’s prosthetic rehabilitation.
Practitioner: Dr. Rebekka Hueber Location: SWISS BIOHEALTH CLINIC | Kreuzlingen Content: During immediate implant placement in region 36, an ankylosed, root canal-treated tooth with apical osteolysis was carefully removed by piezosurgery and an SDS2.0_4614_ba implant was immediately placed in the distal alveolus with very good primary stability. The cervical portion of the implant was augmented with a mixture of autologous cortico-spongy bone material and i-PRF. The patient did not experience any pain or swelling during the procedure or during the healing process.

Practitioner: Dr. Rebekka Hueber Location: SWISS BIOHEALTH CLINIC | Kreuzlingen Content: This immediate implantation with a Balcony implant involved a pulp ankylosed tooth with a pronounced radicular cyst. In only one surgical procedure, the ankylosed tooth including the inflamed tissue was cleanly removed and an SDS2.2_4614_ba implant was placed stably (Anycheck: 74) in the palatal alveolus. Since the pronounced inflammation had breached the vestibular bone lamella, the crestal portion of the alveolar socket was augmented with a mixture of autologous bone, i-PRF, and allogeneic donor bone. Healing was uneventful and without any pain or swelling. Shortly after the procedure, the patient already noticed a significant improvement in his right knee joint complaints.

Practitioner: Dr. Rebekka Hueber Location: SWISS BIOHEALTH CLINIC | Kreuzlingen Content: In today’s observation, an immediate implant was placed after extraction of tooth 37. Due to the absence of a buccal lamella, simultaneous bone augmentation was necessary. With the aid of a sleeve of our Bone Implant Stabilization System (BISS), a retromolar bone plate was easily fixed. In addition, a late implant placement was performed in region 36. Both implants have the special shape “ov-6×8”, which is known as “Double balcony”, to guarantee a perfect base for the prosthetics that will follow later.

Practitioner: Dr. Rebekka Hueber Location: SWISS BIOHEALTH CLINIC | Kreuzlingen Content: GAP3.0 – Guided augmentation protocol of the third generation with insertion of a BISS-Double Cage There was extremely little residual bone, so an external sinus lift was combined with a vertical augmentation of 5-6 mm in the direction of the oral cavity in regio 16/17. The current protocol developed by Prof. Dr. Shahram Ghanaati (University of Frankfurt) uses xenogeneic blocks (Purgo) mixed with allogeneic material and his Open Healing concept. The allogeneic material ensures rapid formation of new bone. The xenogeneic, very slowly resorbing material guarantees high volume stability.

Practitioner: Dr. Rebekka Hueber Location: SWISS BIOHEALTH CLINIC | Kreuzlingen Content: Late implantation in teeth 37/35: According to the CCARD classification for bone defects, this was a class H.I. (horizontal class I defect, low < 4 mm) and therefore the horizontal defect was augmented buccally at the same time, according to the Ghanaati protocol, with a mixture of Purgo, i-PRF and Allodyn. Implant 35 was restored with a digitally fabricated long-term temporary.

Practitioner: Moritz Kneer Location: SWISS BIOHEALTH CLINIC | Kreuzlingen Content: Together with the removal of four FDOKs and two white teeth, a late implant placement was performed in regio 15. Using the Y4 from the AIM system, an SDS 1.2_4611-ov was placed perfectly centrally despite the tight space conditions. In order to achieve maximum stability directly, an internal sinus lift was also performed. The implant was then directly restored with a laboratory-fabricated long-term temporary.

Practitioner: Dr. Josephine Tietje Location: SWISS BIOHEALTH CLINIC | Kreuzlingen Content: After previous mouth-antrum connection (MAC) closure using a PTFE membrane to preserve the soft tissue and bone augmentation through the soft tissue-healed MAC approach, an external sinus lift approach could be avoided. Late implantation followed in region 16. An SDS2.2_4611 was implanted late under ideal conditions, the previously built-up bone was perfectly osseointegrated and optimally perfused. In bone class II, an implant torque of 35 Ncm with an ISQ of 84 was achieved.

JCCI 2021 Content: In this shadowing, an immediate implant placement with ceramic implants of the Balcony special shape was performed in region 16 and 17. Tooth 16, which had undergone endodontic treatment and showed apical periodontitis with subsequent hyperplasia of the basal maxillary sinus mucosa and chronic, painful symptoms, was replaced. Furthermore, tooth 17, which was already trephined and chronically inflamed, was also replaced with an immediate implant.
